Community services are like the superheroes of health and wellness, swooping in to save the day when we’re feeling under the weather or managing long-term health challenges. They make sure we can stay in the comfort of our own homes while getting the support we need. Enter the Care Communities! These vibrant hubs unite health and social care teams to offer a holistic approach that goes beyond just battling illness; they’re all about promoting wellness, encouraging self-care, and inspiring positive lifestyle changes.
Imagine a team of professionals, including GPs, community nurses, therapy experts, mental health wizards, and social service champions, all working together to keep our communities thriving. In East Cheshire, we have five fantastic Care Communities, each with its own flavor: Bollington, Disley, and Poynton (BDP); the dynamic duo of Macclesfield & Out of Hours District Nursing; the cheerful crew from Knutsford, Chelford, Handforth, Alderley Edge, and Wilmslow (CHAW); and the enthusiastic team covering Congleton and Holmes Chapel (CHOC). Their mission? To empower everyone to live independently and healthily for as long as possible, right where they want to be.
But that’s not all! We also have a treasure trove of specialist services that work hand-in-hand with our care communities. Think of Podiatry, the Integrated Respiratory Team, the Palliative Care Team, and others like Tissue Viability and Bladder and Bowel specialists. Whether it’s tackling Long COVID or providing support through Pulmonary Rehabilitation, or even bringing the hospital experience home with Hospital @ Home and Transfer of Care Hub, our community services are ready to lend a helping hand. Together, they create a supportive network that keeps us feeling our best!
Your donations would make a massive difference to our staff in the community to enhance the care that we can provide to our patients. Donations could be used to purchase specialist equipment for example, funding activities for our patients, supporting our volunteers, training our staff, transforming staff and patient areas amongst many more.
